Certified copies of birth or death certificates are available at Shelby City Hall. Certified copies cost $25.00. Certificates may be obtained through our walk-in service or by mail and no appointment is necessary. We accept cash or checks. All questions regarding birth and death certificate copies or spelling corrections should be directed to the Registrar.

The Shelby City Health Department can issue certificates of all births in the State of Ohio.

Death Certificates

Death Certificates are available from 1908. City of Shelby death records before those dates are kept at the Richland County Probate Courthouse in Mansfield, Ohio. Deaths Certificates must be obtained in the city, county or state where the espana med. If you do not know the location of an Ohio death, you may obtain those certificates from the Ohio Department of Health (www.odh.ohio.gov/vs) in Columbus. All original Ohio birth and death certificates are permanently placed on file in Columbus.

Burial-Transit Permit

The Vital Statistics Department issues burial-transit permits to area funeral directors.

Paternity Affidavits

The completion of paternity affidavits is voluntary and acknowledges that a parent and child relationship exists. The affidavit places responsibility for support of the child on the alleged father.
